The Evidence

Our results put SYH’s HOPE Platform in a rare class of technology-enabled solutions with the potential to increase patient engagement and adherence to chronic disease self-management behaviors, and therefore reduce the total cost of diabetes care by reducing acute and chronic complications that lead to avoidable health system use (the major driver of diabetes care costs).

We believe that we have the potential to drive enormous commercial impact, in particular because of our proven ability to engage and manage patients from high-risk groups such as older adults, low-income individuals and those with a high burden of unmet social needs. Table 1 is an example of annualized return-on-investment (ROI) estimator for a real-world sample of adult patients with T2DM with A1c>8% at baseline and average reduction after 12 weeks of DSME of 0.5%. In this ROI estimation, each 1% reduction in A1c is estimated to lead to $16000 in cost saving based on proprietary data from a healthcare payor advisor.
