Our mission is based on the importance of community. To help our community thrive and overcome difficulties managing diabetes, we’ve created an opportunity to engage in one-on-one peer connections: The Buddy System.
How to Join
Click https://emr.seeyourselfhealth.com/openemr/portal to log-in to your patient portal account.
Enter your username and password. Then click “Log In”.
To complete the buddy system profile survey, please click on "Select Documents.” A list should appear with a survey named, “Build Your Buddy Profile” - click on this survey name to open it (don't worry about having the perfect answer to everything, we are interested in any information you are able to provide!).
When you are done, click on the "Submit Document" button at the top of the screen.
We will follow up with you shortly on the next steps on how to connect with your new buddy. In the meantime, you can prepare to message your match by checking out this resource.
